Customer service forms
The most recent forms are kept on this page for three months after publication. Later they are only available on pages specific to each subject.
Some of the forms can be completed on the screen and then printed out. Those forms are marked with an asterisk. The forms are grouped according to the categories used in Finnish aviation regulations. Every form makes reference to the regulation or advisory circular to which it relates. Completed forms should be sent to the Finnish Transport Safety Agency (Trafi) by regular mail.
Only some of the forms have been translated into English.
